The 3,300-square-foot location marks an expansion for the San Francisco-founded company as it continues to build its presence in New York. The space reflects the way PrimaryMD practices medicine: private and designed to accommodate longer visits, movement assessment and on-site testing.

Founded in 2024 by Chief Executive Officer Daniel Yuabov and Chief Medical Officer Dr. Sukhjit “Sarge” Takhar, MD, PrimaryMD brings primary care, prevention, advanced diagnostics, exercise science and longevity medicine together under one physician-led team. The model gives members one place to understand and actively manage their health, with care and health data connected over time and at simultaneously. Dr. Takhar is triple board-certified in internal medicine, emergency medicine and infectious disease and has more than two decades of clinical experience, with training and academic experience spanning UCLA, Harvard and UCSF.

PrimaryMD has established a growing member network in the Bay Area around its approach to health management, and the new SoHo flagship extends this model to downtown Manhattan, complementing PrimaryMD’s existing Hudson Yards location at 503 W 35th St., which will remain open and continue serving members across the West Side.

Located at 430 W Broadway, the new flagship spans approximately 2,000 square feet indoors with a 1,300-square-foot outdoor patio. Designed with a Japanese-influenced, minimalist sensibility, the space has the feel of a downtown pied-à-terre.

With no more than two members in the space at a time, the flagship is designed to feel distinctly removed from the pace and structure of a traditional doctor’s office. The intentional design creates a continuous flow between performance diagnostics, movement analysis, private physician consultations and physical examinations. There is no traditional waiting room; members check in at reception and proceed directly to a private patient care room, creating a more discreet private arrival experience. A hospitality bar, private shower, and outdoor patio contribute to an environment designed with ease and discretion in mind.  

A dedicated area for on-site assessments includes body-composition testing, VO₂ max testing, resting metabolic rate, phlebotomy, movement analysis using the VALD system, grip-strength testing and force-plate testing. These capabilities support PrimaryMD’s broader approach to care, which begins with an in-depth health baseline and continues through ongoing clinical touchpoints. Diagnostic data and health markers are revisited throughout the year to help track changes over time and focus on areas including metabolic health, cardiovascular risk, lipids, cancer prevention, fitness and sleep.

PrimaryMD pairs each member with a dedicated physician-led team consisting of a lead physician, exercise physiologist, medical assistant and care coordinator. Each team serves no more than 150 members, giving physicians and their teams the time and continuity to understand a member’s health history, physiology and individual risk profile. Members have same-day access to their clinical team and unlimited visits throughout the year, with the same team providing continuity across primary care, diagnostics, prevention and ongoing health management.

The SoHo flagship is designed to accommodate up to 450 members across three separate care teams and, ahead of its opening, the location already has an active waitlist.
